Gait training and re-education

Mobility plays a critical role in maintaining quality of life in an elderly individual. Long term conditions, joint stiffness and muscle weakness result in balance deficits which increases the risk of falls and fractures. We help in improving and maintaining mobility in elderly by gait training, muscle strengthening, increasing flexibility, building confidence and promoting postural control.

Pain Management

Chronic pain is common in elderly which impairs the movement significantly. We offer assessment of the cause of pain and effective pain management strategies. This may be done by prescribing appropriate exercise plan, use of manual therapy techniques, modalities, correction of gait pattern and education to improve quality of life.

Falls Prevention

We assess extrinsic and intrinsic falls risks and develop individualised plans working on gait pattern, balance and coordination. The aim is to minimise the likelihood of an injury and maximise the older adults' confidence in their movements. This includes prescription of appropriate aids and equipment, education on creating safe environment at home and falls prevention strategies.

Post-Surgical Rehabilitation

Initial consultation: The process begins with a subjective assessment, where we gather detailed information about the older adult’s medical history, physical condition, and functional abilities. Family members and care home staff play a crucial role in providing this information as it forms the basis of the personalised rehabilitation plan. 

 

Comprehensive Assessment: In the first face to face appointment, a thorough assessment is conducted which includes mobility, muscle strength, flexibility, sensations, co-ordination and  any equipments used. This allows us to identify areas requiring support and tailor our approach to meet the individual’s specific needs.

Building a Customised Care Plan: Based on the information and physical assessment, the therapist develops a personalised treatment plan using a person -centred approach and focussed on the goals outlined by the older adults, family and carers.

 

Ongoing treatment and communication : We pride ouselves with providing hands on intervention which is high quality and based on current evidence. The treatment plan is carefully monitored, reviewed and modified based on the progress and changing needs of the older adults to ensure it remains effective. We are happy to provide regular updates to the family as needed.

Your Therapists

Nicole is an Australian-trained physiotherapist with experience across many hospitals and community settings spanning England and Australia. She has a special interest in neurological rehabilitation, in particular after stroke and spinal cord injury. Nicole is proficient in the use of electrical stimulation, splinting, spasticity and tone management. She is also an APPI trained pilates instructor and incorporates the principles of pilates to improve postural alignment, correct abnormal movement patterns and facilitate motor relearning. Nicole applies a client-specific goal based approach to rehabilitation to make the rehabilitative journey meaningful to each individual.
Ollie graduated from Kings College London with a Postgraduate Masters Degree in Physiotherapy. In addition Ollie also has an Undergraduate degree in Sports Science. Ollie has a wide range of experience, working in Cardiology, Elderly, Thoracic and Urological Surgery Wards. He has a special interest in Acute Respiratory Physiotherapy, Elderly Rehabilitation, as well as extensive experience of working with Paediatric patients in the community. In his spare time, Ollie enjoys cooking, playing Footballs, and going on country walks with his family.
Hanna Rudden graduated with a BSc in physiotherapy from the University of Mälardalen, in Sweden, in 2010. Her career has provided a breadth of experience with time spent in orthopaedics, surgical wards, paediatrics, elderly rehabilitation, MSK rapid response and pelvic health for women. Hanna spends her spare time with her family and friends and when her young daughter allows, she enjoys calligraphy as well as exercising in the gym.
Nic Photiou is an experienced HCPC registered physiotherapist working in the NHS as well as taking on private patients. Armed with a Human Physiology degree from the University of Leeds and a Physiotherapy degree from Brunel University London (2019), Nic has been actively contributing to the field since her graduation. Nic has always enjoyed helping and caring for those around her, driven by passion for health, physiology, and the intricacies of the human body. Nic’s professional focus revolves around the health of the human body, particularly in terms of pathology and recovery from disease and injury. This, combined with her empathetic nature, motivating to assist individuals in reaching their physical potential despite disability or injury. Nic prioritises collaborative relationships within the Multidisciplinary Team and boast experience in diverse areas such as elderly care, falls prevention, community home visits, musculoskeletal injuries, orthopaedics, neurological disorders, dementia, palliative care, and a special interest in respiratory physiotherapy and rehabilitation. Nic employs a comprehensive approach to assessment tools and hands-on-treatments, relying on systemic assessment methods and current evidence, and guidelines to facilitate appropriate management. Her interpersonal skills, a personal strength, are pivotal in building rapport and fostering excellent therapeutic relationships. Residing in North London, her hometown, Nic cherishes spending time outside of physiotherapy with family, engaging in activities such as cooking, gardening, strength training, and hiking.
Toby is an experienced HCPC and CSP registered Physiotherapist. Toby qualified as a Physiotherapist from Kings College London with distinction in 2019. With over 5 years of experience, Toby currently works as a Senior Specialist Physiotherapist within the NHS and Privately. With experience in Musculoskeletal, Neurological, Orthopaedic conditions, and Elderly Rehabilitation including general mobility issues, Toby brings a practical approach to patient care. He is dedicated to developing personalised and holistic rehabilitation plans to enhance the well-being and function of individuals under their care. Through their expertise and commitment, Toby continues to make meaningful contributions to their clients, improving the lives of clients one step at a time.
Venkat has twenty years of experience in providing Physiotherapy assessment and treatment to patients in NHS and private settings. The areas of expertise are Neuro- rehabilitation, Elderly care, musculoskeletal conditions, elective and trauma orthopedics, falls, out of hospital care, post-cancer rehabilitation and palliative care. Venkat completed his Masters in Physiotherapy from Sheffield Hallam University, UK. Venkat had completed Health Education England ‘Quality Improvement fellowship in Frailty’. He has strong inter-personal skills and forms great rapport with patients. Venkat understands the importance of rehabilitation and adapts different approach to get patients back to the previous functional level. He specializes in developing tailor made rehabilitation plan using Evidence based approach. With his pleasant personality and holistic approach, Venkat gets excellent feedback from patients.
Mehmet is an experienced clinician who qualified in Bulgaria and working in UK since 2016. He is highly skilled, and areas of expertise are neuro-rehab, musculoskeletal issues, elderly rehab, post-operative care and general rehabilitation. Along with ‘UKhomephysio’, Mehmet is skilled in devising therapy plans for the individuals with complex conditions and disabilities. He has strong experience in the use of assistive devices like hoists etc. In addition to above, Mehmet has special interest in treatment of musculoskeletal problems, sprains and strains. Experienced Occupational Therapist with a specialist focus on complex moving and handling practice and training, creative problem solving via assessment and equipment provision, formal/informal carer assessment training and facilitating single handed care. As a well a practised therapist Christain is knowledgeable and experienced in the provision of major and minor adaptations for a wide range of clients across multiple disabilities, ages and cultural backgrounds. Clinical experience includes special needs education/support work, inpatient rehab/discharge planning in a hospital setting, case-load management both in hospital and social services settings as well as private sector manual handling training in addition to dealing with complex cases.
A registered physiotherapist in the UK, her expertise is in neurology and neuromuscular conditions in both adults and teens with twenty year’s experience of daily hospital physiotherapy treatments, patients care in Hungary and the UK. Gabriella is experienced in applying the McKenzie method to patients suffering from musculoskeletal disorders. Her areas of expertise are involved mainly with neuro-rehabilitation, especially with stroke victims of all levels and sufferers of musculoskeletal conditions. She is certified in the McKenzie A,B,C,D and E, courses. Gabriella is able to evaluate, treat, and create a preventative maintenance course of action. Her goals of therapy are reducing pain, centralization of symptoms, and a complete recovery from pain. Gabriella is an expert in creating a specialized personal programme which includes education, encouragement, an exercise programme, and self-care treatment. In addition to this, Gabriella has experience in treating neurological conditions like stroke, Parkinsons disease etc. She has goal focussed and holistic approach which has benefitted patients immensely. Raj qualified as a Physiotherapist in 2000 and worked internationally before moving to UK in 2005. Since then, Raj been providing physiotherapy specialist assessment and treatment both NHS and Private patient’s in clinic and home settings. Raj is strong believer of life long learning and his expertise includes in the field of Musculoskeletal conditions, Orthopaedics and trauma, Post discharge care, palliative and neuro rehabilitation. He understands the client focussed intervention and he tailors his approach for each individual with evidence based intervention depending on their needs and preference. Physiotherapy intervention to relieve chronic back pain is his special area of expertise and he has helped numerous patients with promising results.
Vinod qualified in India as a Physiotherapist in 2000 and started working with NHS in 2007. He has experience in working in a variety of different specialties within the NHS as well as the private sector over the years in junior and senior level. Specialties include musculoskeletal, neurological, rehabilitation, care of the elderly, falls prevention, balance training, orthopeadic conditions and community rehabilitation. Neuro-rehabilitation included work with patients diagnosed with Parkinson’s disease, stroke patients and Multiple Sclerosis etc. He is skilled in rehabilitation of elderly patients with generalised deconditioning, falls, joint replacement, fractures and reduced mobility. As well as working for UKhome Physio , Vinod currently works as a senior Physiotherapist with NHS in a community rehabilitation team.
Bobby has been a Physiotherapist for last eight years with experience of working in multi-disciplinary teams across NHS and private sector. His areas of expertise are rehabilitaion following orthopedic surgeries like backs and joint replacements, muskuloskeletal conditions, stroke, amputations etc. He is competent in developing and implementing the rehabilitaion plans. Mani has seventeen years of experience in providing Physiotherapy assessment and treatment to patients in NHS and private settings. Having worked in both wards and community, she understands the importance of rehabilitation needed to get patients back to the previous level of function and mobility. Strong inter-personal skills and a caring attitude has given her the ability to strike great rapport with patients. Mani’s approach is holistic thus, treating patient to achieve optimal health. In addition to providing treatment for above conditions, Mani is also skilled in therapeutic massage to relieve spasms and pain. Evo has been a qualified physiotherapist since 2014. His expertise is in neurology and neuromuscular conditions in children and young adults. Evo is a full-time research physiotherapist, and an author of several papers in neuromuscular diseases. He has presented his work at numerous events and academic conferences around the globe. Evo graduated with Master’s degree in paediatric physiotherapy from University College London in 2021. Apart from academia, he is regularly involved in the outpatient clinics at a tertiary NHS hospital where he works. His other interests are in sport and sport injuries and prevention, exercise physio, balance and falls prevention. During the current senario , Evo volunteered for 3 months at Royal Free hospital and worked in the major pandemic Intensive Care Units. In his spare time, Evo is a keen trail ultramarathon runner, mountain hiker and climber.
Tosin has over 21 years of experience working as a physiotherapist in NHS hospitals, private physiotherapy clinics, community rapid response and intermediate care environments. She specialises in rehabilitation within patients’ own home. She is very experienced in assessment and treatment of a wide range of conditions like,falls, orthopaedics, neurological disorders, palliative care, dementia, musculoskeletal issues etc. Tosin is able to use a wide range of physiotherapy assessment and clinical reasoning skills in order to diagnose and formulate an effective treatment plan for her patients. Her approach is evidence based which tends to ensure goals setting with patients is SMART to help manage and establish patient expectations for a positive outcome.
